Mandated reporters are individuals who are legally required to report known or suspected abuse and neglect of vulnerable populations to state and/or local authorities. Laws surrounding mandated reporting vary from state-to-state so please ensure your familiarity with your state's laws regarding individuals who are required to report known or suspected abuse and neglect.

Child abuse and neglect background checks may also be referred to as Adam Walsh checks, CANTS checks, maltreatment checks, child abuse registry checks, or 689 background checks.
To begin the background check process, the applicant must complete the CFS 689 Authorization For Background Check for Programs not Licensed By DCFS.
Who should fill out the 689 form?
- Illinois community mental health or developmental services agencies that are required to obtain a child abuse and neglect background check under Paul’s Law.
- Organizations required by an Illinois state agency, other than Illinois DCFS, (IDPH, IDHS) that requires a child abuse and neglect background check.
- Schools not licensed by Illinois DCFS.
- Nonprofits and other organizations that are not licensed by Illinois DCFS.
- Out-of-state licensed day cares and other organizations required by their state’s law to obtain a child abuse and neglect background check.
- Out-of-state placements, foster care, adoptions.
- Organizations that want to implement child abuse and neglect background checks for their employees, volunteers, or vendors.
