Arizona Public Sector and Service Providers:
For employment/volunteer purposes, please use Arizona’s new Centralized Background Check System. This system requires both the employer and prospective employee to register, and the prospective employee is responsible for selecting the prospective employer to receive the results of their background check. Fingerprint clearance status updates (DPS), Adult Protective Services (APS), and Department of Child Safety (DCS) background check results are all included. Visit https://des.az.gov/cbc(link is external) to register as an employer or prospective employee and request a background check.
Out of state requests must continue to use: DCS-1083A form and email the completed 1083A form to DCSCentralRegistry@azdcs.gov.

Arizona Adult Protective Service (APS) Registry
The public facing APS Registry contains the name and date of birth of the person determined to have abused, neglected, or exploited a vulnerable adult and includes a description of the maltreatment which occurred.
The purpose of the APS Registry is to prevent vulnerable adults and children from being victimized by individuals who have been found, through an APS investigation and due process, to have abused, neglected, or exploited a vulnerable adult.
All employers are encouraged to review the APS Registry when deciding whether to employ a person to provide care for vulnerable populations.
Owners of residential care institutions, nursing care institutions, and home health agencies, are required to check for a prospective employee on the APS Registry and prohibit the hiring of employees who appear on the Registry (A.R.S. § 46-459).
Owners of residential care institutions, nursing care institutions, or home health agencies must verify that each employee is not on the Adult Protective Services Registry and shall take action to terminate the employment of that employee should they appear on the registry.
Annual reverification that each employee is not on the Adult Protective Services Registry is required pursuant to Section 46-459.
